2011 - 2012 SCHOLARSHIP APPLICATION

~ REQUIREMENTS ~

· Open to any High School Senior in the 19^th Masonic District

· Minimum of a 3.0 GPA (grade point average) required

· Student will be attending an accredited institution of higher learning

· A brief essay explaining why student wants to go to College or

Professional School

· The student must enter the College within one semester/quarter

(excluding summer school) after being awarded this scholarship

· All information on application must be completed

· Parent’s (Guardian‘s) and student’s signatures are required on application

· Deadline for receiving application is Friday, March 23, 2012

All applications and essays will be judged on their own merit without regard

to race, sex, creed or religion
